What is a touchpoint?

A child’s brain undergoes rapid development during the first three years of life, making this period crucial for their overall growth. Each new brain connection formed during this time has been referred to as a “touchpoint,” which is why we named our informational webinar series after this important milestone in a child’s development. Through these hour-long, interactive webinars, our goal is to help you create your own touchpoints across South Dakota, contributing to the healthy development of all the state’s children.

Advancing Pediatric Medical Homes in South Dakota: Building Coordinated, Family‑Centered Care for Children

This session will provide an overview of the Child Medical Home model and how lessons from it can support coordinated, family-centered care for children across South Dakota. The presenter will explain the core components of the AAP Medical Home model, highlight South Dakota’s current strategies, and share practical examples of how families, providers, and community partners can strengthen early and sustained connections to a medical home. The session will also review state-level data from the National Survey of Children’s Health and outline opportunities for cross-sector collaboration.

Learning Objectives:

As a result of attending, participants should expect to be able to

• Describe the core components of the AAP Child/Family-Centered Medical Home model.
• Identify how clinical partnerships and effective care coordination can support Medical Home implementation statewide.
• Interpret key Medical Home indicators from the National Survey of Children’s Health (NSCH) and understand their relevance for child health outcomes.
• Apply practical approaches for strengthening family-centered care, care coordination, and early medical home connections in clinical and community settings.
